What we collect
We collect your name, contact details, date of birth, and health information including medical history, injury details, and treatment records. This information is collected when you book an appointment, complete an intake form, or attend our clinic.
How we use it
We use your information to provide and manage your care, process payments and insurance claims, follow up with you after appointments, and refer you to other practitioners where clinically appropriate.
Who we share it with
Your information may be shared with treating specialists or allied health practitioners involved in your care, Medicare, your private health insurer, or WorkCover where applicable. We do not share your information with third parties for marketing purposes.
How we store it
Your records are stored securely within Cliniko, our encrypted practice management system. Access is limited to authorised Colab Physio practitioners and staff
Your rights
You have the right to access and correct your information, and to make a complaint if you believe your privacy has not been handled appropriately. Complaints can be directed to the Office of the Australian Information Commissioner at oaic.gov.au.
